Support Coordination is funded under Capacity Building – Support Coordination in your NDIS plan.

Our Support Coordinators will assist you to build the skills you need to understand, implement and use your plan. This includes:
- Building your capacity and ability to design, develop and implement your supports
- Helping you understand your plan and how to utilise the supports that are available in your plan
- Assisting with accessing the NDIS portal
- Linking you with the most appropriate services to help you reach your NDIS goals
- Coordinating services in your community to best meet your needs and goals
- Developing service agreements with service providers, including obtaining quotes for supports to meet your needs
- Coordinating agreed services
- Resolving any issues or conflicts if they arise
- Monitoring your plan outcomes and providing appropriate reports
- Preparing for your plan review
